The Immigration Department of Barbados says the passport application form «should be accompanied by two (2) photographs», «not more than five (5) centimetres by five (5) centimetres» and «taken by a professional photographer». The applicant signs the boxes at the bottom of page two of the form and the declaration in section 11 on page four, and «the form and one (1) photograph must also be certified by a person authorised to act as a guarantor».
