For a Vietnam work permit the file lists «02 colored photos (4cm×6cm, with white background, frontal side, bareheaded, and no colored glasses)», and the photos must be «taken within 06 months of the date of application» — the Viet Nam government portal puts this line in the document list next to «01 notarized copies of the valid passport». So the photo is a 4 × 6 cm colour print, a tall format, on a white background, facing the camera, with no head covering and no coloured glasses; the National Public Service Portal procedure (checked earlier, not readable today) adds a clear face and clear ears, and no glasses at all, so take the glasses off and you satisfy both wordings.
You need two photos. The work permit is issued by the Ministry of Labour – Invalids and Social Affairs or by the provincial or municipal Department of Labour (the one that matches the employer's type), and the conditions come from Labor Code 2019, Article 151.1: at least 18 years of age, qualifications and experience, suitable health and no criminal record.
